Perlane® is a hyaluronic acid-based dermal filler. The injectable gel is used to smooth out and fill in moderate to severe creases and lines around the nose and corners of the mouth (nasolabial folds, also known as marionette, smile, and laugh lines). It works by plumping up the skin and giving it fullness, which reduces the appearance of lines and creases. What makes Perlane® different from other hyaluronic acid-based fillers, such as Restylane®, is particle size and injection depth. Perlane® is formulated with larger particles and greater injection depth. This makes it ideal for use on deeper wrinkles. Perlane® is indicated for use on moderate to severe facial wrinkles and depressions, including smile lines, acne scars, and hollows beneath the eyes. Perlane® is safe for most adult patients under the age of 65. However, patients should not use it if they are allergic to lidocaine or have a history of severe allergies or anaphylaxis (inability to breathe) that required hospitalization or if they have a bleeding disorder.
The Perlane® Procedure
The Perlane® procedure is quite simple and can be done in the office. The product is injected into lines or creases using an ultrafine needle. For patients apprehensive about discomfort during treatment, numbing medication can be applied to the treatment site.
Patient recovery time is minimal after injection treatment. Avoid using St. Johns wort, high doses of Vitamin E, or aspirin or other NSAIDs (such as ibuprofen) two weeks before and two weeks after treatment. These will all increase the patient’s risk for bruising or bleeding at the injection site.
Apply a cold compress to the injected area immediately after treatment to reduce swelling. Do not touch or rub the treated area for six hours following treatment. After that, the area can be gently washed with soap and water. Avoid exposure to intense heat, such as from a sun lamp or sun bathing, until the redness and swelling disappears.
Perlane® Side Effects
The most common side effects include redness, swelling, and bruising at the injection sites. These are generally mild and will start to disappear about a week after treatment. The use of cold compresses and Arnica montana will reduce the amount of swelling and bruising.
A darkening of the skin, or hyperpigmentation, may occur in some patients; this may last for several weeks. Another risk includes the possibility of infection. Perlane® is not associated with a risk of allergic reaction, however, since the product contains natural ingredients that are naturally found in the body.
Patients should start to see results within two to three days following the Perlane® procedure. Studies have shown that 87 to 92 percent of patients felt that Perlane® had decreased the appearance of their facial creases at three months after treatment. At six months after treatment, 63 to 71 percent of patients were still satisfied with the results of Perlane® treatment.
